The Delaware Division of Substance Abuse and Mental Health (DSAMH), in partnership with Bamboo Health, announced the rollout of the Delaware Treatment and Referral Network 360 (DTRN360) behavioral health care coordination platform, which is designed to connect provider organizations, improve care transitions, and reduce silos across the behavioral health system. DTRN360 is the result of a multi-year effort to bring together treatment provider organizations, state agencies, and care teams through a unified digital platform supporting real-time communication and care coordination. More than 180 organizations have joined DTRN360.

DSAMH launched the DTRN in 2018, using Bamboo Health’s OpenBeds technology. Over 250,000 referrals have since resulted, according to the announcement. In 2023, DSAMH expanded the partnership, adding Bamboo’s Crisis Management System. These tools were the foundation for the full statewide launch of DTRN360.

The state chose two organizations to build out the care coordination solution: Bamboo Health, for the single integrated platform, and FindHelp for social determinants of health (SDOH) referrals, which will be accessible within the integrated DTRN360 digital platform. DTRN360 is expected to provide participating organizations with the following benefits:

• See relevant health and behavioral health history for consumers
• Work within existing clinical workflows
• Receive real-time alerts
• Track consumer referrals and placements across the care continuum
• Connect consumers with services addressing SDOH, including peer support, housing, food insecurity, and other community-based programs
