Epicured Inc., a “food-is-medicine” organization dedicated to combating and preventing chronic disease, announced a partnership with Fallon Health, which offers the Program of All-Inclusive Care for the Elderly (PACE)—a comprehensive, community-based model of care that helps older adults remain independent and in their homes for as long as possible. Together, they provide Medically Tailored Meals (MTMs) to participants of Fallon Health’s Massachusetts PACE, locally called Summit ElderCare. The meals are used as a therapeutic tool to enhance care, improve health outcomes, and decrease health care costs for adults with chronic health care needs, the announcement stated.
The meals are available at four out of five of Fallon Health’s Summit ElderCare PACE Centers, with plans to expand the offering to additional locations. This initiative serves approximately 1,000 MTMs per week, supporting hundreds of older adults enrolled in the program, many of whom face food insecurity or complex health conditions.

Fallon Health supports approximately 1,700 participants and their caregivers. It operates five PACE Centers in Massachusetts through its Summit ElderCare program and an additional center in Western New York through Fallon Health Weinberg-PACE.
