The Hawaii Department of Human Services (DHS) released a request for proposals (RFP-MQD-2026-001) to rebid its Community Care Services (CCS) Program behavioral health organization (BHO) contract for statewide management of adult community mental health services. The incumbent contractor, Ohana Health Plan (a Centene subsidiary), has held the contract since 2013.
The RFP was released February 13, 2026, with proposals due March 20, 2026. The new contract is scheduled to begin July 1, 2026, and run through June 30, 2029, with three optional one-year extensions. The contract is valued at approximately $60 million annually.
The CCS program serves Medicaid beneficiaries age 21 and older with serious mental illness (SMI) or serious and persistent mental illness (SPMI) who are enrolled in Hawaii’s QUEST Integration (QI) managed care plans. Individuals must first enroll in a QI Health Plan before electing participation in CCS. Enrollment in CCS is voluntary, and approximately 5,000 beneficiaries participate statewide.
The selected BHO will manage and coordinate intensive and basic behavioral health services and ensure integration of behavioral and physical health care.
Effective January 1, 2026, DHS began implementing a phased rollout of reentry services. The CCS BHO will be required to operationalize processes supporting reentry services, including eligibility determination, enrollment coordination, care management, and post-release service delivery.
The BHO is paid a capitated rate across four member categories:
- Medicaid-only beneficiaries under age 40
- Medicaid-only beneficiaries age 40 and older
- Medicare-eligible beneficiaries under age 40
- Medicare-eligible beneficiaries age 40 and older
